$135 for 45 minute appointments (CPT Code 90834)
$145 for 60 minute appointments (CPT Code 90837)
$160 for initial intake assessment appointments (90-120 minutes; CPT Code 90971)

Sliding Scale and Pro Bono Rates (availability may vary)
I offer sliding scale services with fees as low as $60 per therapy session, sliding at increments of $25. Individuals can choose from these rates of $110, $85, or $60 based on individual/family financial need. Individuals with income less than 200% of the Federal Poverty Line quality for significantly reduced rates or free services depending on income. Please note that these rates are limited by slot quantity and do not have guaranteed availability.

Sliding Scale Private-Pay (Cash, Credit Card, HSA/FSA Card, or Check)
I offer a “Pay What You Can” sliding scale from $60 - $110 for individual, relationship, and family counseling sessions. You are the expert in your financial situation, and I trust that you can choose the sliding scale rate that works for you, from options of $60, $85, or $110 per hour long session. Please note, your initial intake appointment will be charged for $25 more than your designated sliding scale fee to account for the longer appointment. Availability may vary by pay rate.

Insurance & Out-of-Network Billing
I am currently in-network with Utah Medicaid, University of Utah Health Plans, Molina Healthcare, Regence Blue Cross Blue Shield, and SelectHealth insurances. If you would like to bill your services as out-of-network (OON) to your insurance company for reimbursement, I am happy to provide a monthly superbill of services. If you are Out-of-Network, you also may be able to pursue a Single Case Agreement with your insurance provider.

Low-Income Options
If your household income is at 200% of the Federal Poverty Line (FPL) or below, you may qualify for a limited sliding scale rate spot. For individuals under 100% of the FPL, therapy rates are free. For individuals under 200% of the FPL, therapy rates are $10 per individual, relationship, or family session, $15 for intake, and $5 per group. Please see below for the Federal Poverty Line chart to see if you qualify. Please note, verification of financial hardship will be required and slots are limited.
